1. Go to www.work.illinois.gov
2. Go to FORMS look for: Employment Application CMS100: Fillable version for online application.

To capture the PEL from Illinois State Board of Education (ISBE):

1.  Go to www.isbe.net/ELIS  
2.  Click on PUBLIC SEARCH
3. Type in your name and print the PEL Information on the screen.
